Jazz Cafe Alto near Leidseplein
Maloe Melo, short walk West from Leidseplein. Rock and blues, loud AF in that back room.
Gollems Proeflokaal on Overtoom, jazz on Saturday evenings but get there way early.
Waterhole, just off Leidseplein, mostly rock, afternoon happy hour for 3.50 pints 14:00-19:00.
I'm sure others have more suggestions.

Two other (very different) suggestions could be Bourbon Street jazz club on Leidsekruisstraat (not smoker's friendly IIRC, but nice little place with her good music just around the corner from Easy Times where you can go to smoke) and Kashmir Lounge (which has some really nice live dj sets and such, and is most definitely smoker friendly).
There are many, many options but sadly not as many now that are smoker friendly, you could probably get away with vaping a quite a few places if you have a pax or some other stealthy gear...

One other place, somewhat hit and miss:
At the top of Zeedijk there is a funky restaurant named "Skek". They used to run music on Saturday, but finish by 22:00. If you're in the area check it out.

I would agree that Waterhole is a good recommendation, was there last night, even on a weekday the place was full and had a bunch of cool homies playing pool and dancing around in circles.
